Imaging mass spectrometry diagnostics
Label-free maps of proteins, peptides and lipids across a tissue section, read with a laser at every pixel and matched to histology — emerging for clinical laboratories, with a CAP checklist since 2019.
1 · What it gives
Imaging mass spectrometry (MSI/IMS) is a label-free technique that maps the distribution of proteins, peptides, lipids and other compounds across a tissue section by measuring mass spectra at each pixel. For protein distribution it is positioned as an alternative to immunohistochemical staining, since it needs no labels. It is described as emerging for clinical laboratories and not yet turnkey: it needs standardised protocols and strict controls. Over 1,000 publications a year show it has entered mainstream analysis methods. One early-detection use case is histology-guided MALDI IMS of bronchial biopsies from high-risk people.

3 · How to implement 4 steps
- 1Prepare tissuePrepare the tissue and deposit matrix uniformly (for MALDI).
- 2Acquire spectraRaster a laser over the tissue to record a full mass spectrum at each pixel.
- 3Build mapsConvert spectra to 2D molecular maps and correlate them with microscopy and histology.
- 4Validate for clinical useDefine the assay workflow, quality metrics and reproducibility before regulatory submission.

United States
In the US clinical setting, the College of American Pathologists (CAP) added an IMS-specific revision to its Chemistry and Toxicology checklist in 2019, covering instrument calibration and performance evaluation, control tissues and analysis requirements. Regulatory approval needs a clear workflow and defined analytical validity, clinical validity and clinical utility.
European Union

7 · Tech stack & innovations
Main risks from the sources: clinical labs need stringent controls on sample preparation, data quality and bioinformatics to withstand regulatory scrutiny; throughput and turnaround must match routine diagnostics; approval needs rigorous assay performance definition.
